Employers use this form if they are required to pay state unemployment tax in more than one state, or if they paid wages in any state subject to credit reduction. This form must be attached to the 940 when filed. This new form will:

Ease credit reduction computations
Increase the accuracy of return processing
Reduce the need for taxpayer correspondence with the IRS NOTE: Not all 940 users will need this new 940 Schedule A
