This 4-part carbonless form includes employer copies A (federal, red scannable) and D (file), and employee copies B (federal) and C (file). Your typewriter or pin-fed printer will print all copies at the same time. Forms are 8.5" wide (format changed in 2001). Mail employer copies in a batch to the SSA.
