Use this W-2 form to report wage and withholding information. This 6-part carbon-interleaved form includes a set of forms for employer copies A (federal red scannable), 1 (state) and D (file). A second set of forms for employees includes copies B (federal), 2 (state) and C (file) which are pre-inserted into an envelope. When you use your typewriter or pin-fed printer, the mailing address will also be printed on the envelope. You just seal and mail! Forms are 8.5" wide (format changed in 2001). Mail employer copies in a batch to the SSA. Employee copies are ready to mail.
