THIS IS THE OFFICIAL IRS VERSION of the 1095-C and should be mailed to the employee, as well as filed with the IRS with Form 1094-C transmittal to summarize the batch of 1095-C forms for a single employer.

If you are mailing forms instead of e-filing with the IRS, be sure to order 2 forms for each employee.

1095-C Forms for ACA Health Care Coverage Reporting – Official IRS Version

ACA Form 1095-C is used by employers with 50+ full-time employees (or equivalent) to provide their employees with documentation outlining any applicable offer of health coverage.

You will need 2 forms per employee: one for the IRS and one for the employee. If e-filing, order 1 form for the employee only.

CONTINUATION FORMS MAY BE REQUIRED!

Form 1095-C (IRS Landscape format) is now two pages. If the employee has  dependents who were enrolled in coverage, you must purchase the 1095 Continuation form to maintain compliance.
