This full-sheet form is for use with ComplyRight ACA Software and Laser Link Software only. It is a full page with the form pre-printed at the bottom.

If an employee has additional dependents covered under the plan, use 1095-B continuation forms.

You will need 2 forms per employee: one for mailing to the employee, and one for the employer file copy.

Don’t forget compatible 1095 envelopes!

Use Form 1094-B transmittal to summarize information on 1095-B forms when mailing to the IRS.
